Can I Ask Staff to Complete a Self-Certification Form?
You can choose a self-certification form to be sent to staff when they take 7 days or less off in once sickness instance. This is sent to them when they return to work and requires them to provide the following information:
When the self-certification form has been completed, this is saved against the sick leave event held in your calendar. The reason for absence field on this form is mandatory and will be applied to the sick leave event, so if staff fail to provide this information at the time they report their sickness absence, this information will be updated once the self-certification form is submitted.
To set a requirement for the self-certification form to be sent to staff, go to your Company Settings > Sickness and tick the option to request self-certification. forms when a member of staff takes sick leave.
